(1) 工廠檢查申請書。(Application form of factory inspection)

(3) 工廠位置圖。(Location map of factory)

(5) 製造流程圖。(Manufacturing flow chart)
(6) 檢測方法概要。(Brief inspection/testing method)
  n; H6 {4 A2 {7 R(7) 產品之型式試驗報告或已申請型式試驗之證明文件。(Type test
report or already test applied proof)
(8) 代理國外生產廠場申請工廠檢查者，應檢附委任書及證明文
件。(Letter of Authorization for Application of factory inspection

(10) 經濟部標準檢驗局或其認可之CNS12680 系列品質管理系統

certification body)
* ~* q7 m' F. r' u5 p2 a0 Q& O前(3)~(9)項若為外文者，ETC 得要求申請人加附中文譯本。
(ETC may ask for document with Chinese translation if document for item
3-8 are in foreign words)
(11) 項非必要文件，但若提供的話，得免檢查部分項目。

(三) 受理時，即需核對申請資料，有不齊全時可視情形於執行初次工廠檢
查前提供給本中心。(We’ll review all your sent document when handling, if not
+ Y% R3 s4 t2 H$ a3 ^complete, we’ll not establish project, however in some cases, we can accept
establishing this application, but applicant must make it complete before our factory
inspection execution)

4.檢查人天原則視工廠申請範圍多寡/人數等情形而定，並經客戶確認。

(10) 經濟部標準檢驗局對個別商品種類訂定之工廠檢查特定要

6.初次工廠檢查結果符合規定者，將發給工廠檢查報告。(Inspection certificate will

7.以工廠檢查報告取得商品驗證登錄之生產廠場，須接受後續工廠檢查的安
9 a& c+ G6 i3 e: T+ x  M排，原則上每年至少一次，但情況特殊者得增加檢查次數。(Factory registered

8.後續工廠檢查時應執行取樣，查核與驗證登錄模式二原型式之一致性，必要
時進行測試。(During performing follow up inspection, sample request is required to
